Sign-Up Form/Permission Slip

Please fill out both forms entirely and either deliver them to the Board of Education office at the south High School entrance or mail to:

Farwell Area Schools, c/o FEYFA, 399 E. Michigan, Farwell, MI 48622.

7th and 8th grade students must also turn in a physical card with both forms.

Rates are as follows: $50 for the first child, $40 for the second, and $30 for each additional child in your family. Make checks out to FEYFA.

Separate paperwork must accompany each child.
